History and clinical signs A12 yearoldneuteredmaleBelgiumShepherd dog (26kg)waspresentedwithpolyuriaand polydipsia, increasedappetiteandexercise intoleranceof fewweeksduration.Despite normalmaintenance rations, thedoggained about5kgofbodyweight.Onphysical examinationobesitywithapot-bellied appearanceandmuscleatrophywereobserved. Diagnostic work up Thediagnosticworkupofpolyuriaandpolydipsia was startedwithurineandbloodanalysis.The urinehada low specificgravityand themain biochemicalabnormalitywaselevatedalkaline phosphatase.Basedonhistory,clinical signsand initial laboratoryfindings,hyperadrenocorticism orcanineCushing`s syndromewas suspected.

Thediagnosiswasconfirmedbymeasuring theurinarycorticoid:creatinine ratios (UCCRs) combinedwithahighdosedexamethasone suppression test (HDDST),andendogenous ACTHconcentration.TheUCCRswerewell over the reference valueof10x10 6 and the suppressionofcortisolbydexamethasone administrationwasmore than50%.These resultswere indicative forpituitary-dependent hyperadrenocorticism.Mainlydue to theageof thedog, theowner refused furtherevaluationof thepituitaryglandwithdiagnostic imaging.
